This form certifies that the client will use the background information contained in the report for pre-employment screening purposes only. Federal law requires that SI keep this signed form on file.

Pre-Employment Investigation Search Request Form 
Use this form to request pre-employment investigations. This form should be faxed to us along with the Notification and Release Authorization, the candidate's job application and resume.

Notification and Release Authorization 
This form must be signed by the job applicant and faxed to us along with the above listed forms.

Fair Credit Reporting Act Summary 
This form is a summary of rights that must, by law, be presented to the applicant at the time the Notification and Release Form is signed.
